Amit Shah unveils PM Family Care Tracker and Health Passport in Gandhinagar

Union Home and Cooperation Minister Amit Shah on Sunday launched the pilot project of the PM Family Care Tracker (PM-FCT) and Health Passport for the Gandhinagar Lok Sabha constituency in Gujarat. He described the initiative as an important step towards transparent and technology-driven governance. He said the platform will help ensure that no eligible beneficiary misses any government welfare scheme.
Addressing the gathering, Shah said a new phase of welfare for the poor started in India after 2014. He highlighted the role of technology in improving governance and making welfare delivery more efficient.
“Under the Prime Minister’s transparent governance, every rupee released from Delhi is now deposited directly into the bank accounts of the poor,” he said.
Shah said India has witnessed major reforms in several sectors under Prime Minister Narendra Modi’s leadership. He said the government introduced long-term planning in education, the economy, industrial development and governance. He added that transparent systems now allow welfare benefits to reach beneficiaries directly through bank transfers without any middlemen.
He also said, “Whenever the history of India’s first 100 years of independence is written, it will be divided into two phases—before 2014 and after 2014.”
Speaking about the newly launched pilot project, Shah said the PM Family Care Tracker brings together information related to birth and death registration, health, nutrition, education and social welfare on one digital platform. He said the system will create a common identity framework that will strengthen governance and help authorities identify people who are eligible for government schemes.
Shah explained that the platform will automatically generate alerts if a child misses a vaccination or drops out of school. The system will send notifications to local officials, MLAs and MPs. Volunteers and government authorities can then take immediate action to help the concerned family. He added that the platform will digitally monitor the health and nutritional status of children from the prenatal stage until they turn 16 years old.
Recalling his visits to villages in Uttar Pradesh, Shah said he had personally witnessed the hardships faced by poor families. He said the Modi government improved the lives of nearly 70 crore people over the past 12 years through several welfare programmes. These initiatives include housing, electricity, tap water, toilets, LPG connections, free food grains and free healthcare worth up to ₹5 lakh.
“These initiatives had fulfilled the dreams of generations of poor families,” Shah said.
Referring to Mahatma Gandhi’s vision of Ram Rajya, Shah said good governance is not limited to introducing welfare schemes. He said it also means taking responsibility for the well-being of every citizen and ensuring that government support reaches every eligible family.
Gujarat Chief Minister Bhupendra Patel also addressed the event. He described the PM Family Care Tracker and Health Passport as a well-planned and people-centric digital health infrastructure for every family. He said the initiative marks the beginning of a “new era” in providing safe, healthy and dignified lives to mothers and children from pregnancy until adolescence.
Patel said the state government continues to focus on four pillars of healthcare—preventive, promotive, affordable and digital. He also highlighted Gujarat’s achievements under schemes such as the Namo Shri Yojana. He said these initiatives have strengthened healthcare services across the state and improved access to medical support for mothers and children.
During the event, Shah distributed the first digital certificates issued under the new system. He also administered polio drops to children, distributed nutrition kits to beneficiaries and issued Health Passports. In addition, he inaugurated the PM eBus Sewa in Gandhinagar, adding another public service initiative to the day's programme.
The PM Family Care Tracker and Health Passport pilot aims to create a connected digital system that follows the health, education and welfare journey of every family. The government expects the platform to improve service delivery, identify gaps in welfare coverage and enable timely intervention through better coordination between departments and local authorities.
